The Intake Process
*Contact AHC via email, the web contact form, or by requesting an appointment online.
*You will be sent intake paperwork via email. The forms require an e-signature and once completed will automatically be sent to your AHC file.

*Schedule with your AHC therapist.
*Your card will be charged a $30 fee to hold the appointment time you selected.
* This fee will not be refunded should you not show for your appointment. If you do show the fee will be applied to your out of pocket expenses.
